  • Identify hazards (Hazard reporting, Take 5, JHA, WPI, internal & external audits) and take actions immediately to prevent potential incidents
  • Establish metrics to measure everyone’s contribution (leading indicator such as Metrics system)
  • Learn from others’ mistakes and prevent (safety alerts + actions)
  • Comply the legislation to improve workplace standards
  • Lead by example at all levels (My and others’ safety is part of my job and I have a duty of care)

Control & Blame Culture:

  • Take actions when incident happened (lagging indicator)
  • Wait accidents to make the changes
  • Get prosecution and comply the legislation
  • Learn from own mistakes
  • Safety is Safety Department’s job. They shall do Hazard report, Take 5, WPI, JHA, Incident Investigation and all necessary things for my safety.
  • HSE Department is policemen who control and blame everyone who involved in incidents.
