Take 5 – Personal hazard identification tool

Its pocket booklet which contains pre-start check 10 questions at task and hazard report sheet. Since we use own developed Take 5, our incident (event) number had been decreased significantly as shown below on graph. This is one of the secrets of our employees are working LTI free for 548 days since 13th May 2014.

On the other hand, it’s a personal tool to change a personal behavior and a reminder what necessary to be done before any task that is used mostly at remanufacturing, workshop, welding and warehouse (high risk) areas of our Company.

You may develop it further based on your business orientation and create own Take 5. Remember:

  • Each question has background survey (previous incident reports, hazards and common risks at industry) and philosophy. So before changing the questions please make a survey what is actually real & common risks to your employees – hit your target.
  • Risk matrix can be changed according to your business specification.
  • The greatest challenge is implementation. Training shall be mandatory and Leadership at all levels will be required. Especially Top level support should be essential.

Second section of this booklet is hazard report sheet. Whoever identifies any hazard wherever, writes on this sheet and gives to HSE staff. HSE staff sends immediate notification to relevant Supervisor to control this hazard and registers it into Corrective Action Report (CAR) that is sent to All Directors, Managers and Supervisor every Monday at 15:00pm.
